I'm looking at two days in Tokyo (with two primary school kids).

What would people recommend?
If you haven't been to Tokyo before then you have to spend one day doing siteseeing tour of best of Tokyo. I would skip the fish market if you can avoid it and go shopping in Shibuya instead. There is a great Pokemon center and Nintendo store for kids.

If it is all about keeping the kids entertained, i suggest taking them to Disneyland for one day. And if they like Harry Potter, there is a studio tour you can take them as an alternative.

Ueno zoo. Zauo fishing restaurant. sumo (if its on). various attractions on odaiba. Tokyo dome city theme park. game centre at akihabara or shinjuku.

plenty on this thread about kidzania if they are 5-8
absolutely a must with kids.

Mine were 7 & 5 last year when we went

Its also a short walk to Teamlabs which makes it a good whole day trip

Ueno zoo.
Ueno zoo was ok.

The pandas and polar bears are cool as was the indoor reptile centre with the salt water crocodile

The rest of the park felt very cagey to me

Got the itch to head back again, went in 2015 and it remains my favourite trip, did Tokyo, Osaka, and brief periods in Hakone and Kyoto and Hiroshima.

Would do another week in Tokyo, and then in the other week some new places, I'd probably do one of the other major cities like Yokohama, Sapporo or Fukuoka , and then maybe a couple of places a little more remote/ country for something different.

I'm travelling to Tokyo next April for the first time.

I'm interested in baseball, J League, horse racing, Tokyo Disneyland, Japanese rock bands and collecting vinyl.

Expecting big things, so any additional suggestions of things to do would be welcome

Second the food.

Of those things I only have experience with Tokyo Disneyland, it's great, but it's effectively any other Disneyland in Japanese.

Tokyo DisneySea we also went to, and is similar but I believe unique as there aren't any other DisneySea in the world, well there weren't when I went in 2015.

Mt Fuji and Lake Ashi is a really cool daytrip, as is a trip out to Kyoto by train, as is a trip out to Hakone the Onsen district. These aren't on your list of interesting things but are other things we enjoyed to get an experience of Japan.
