I don't remember which team but I remember a few years ago a free kick was called and the umpire turned around to see the ball coming back very badly to the player who won the free and the umpire immediately called a 50 metre penalty not realising that it was in fact a team mate that threw the ball back badly and not an opposition player.

No where near as bad as Harvey though as what Harvey did was deliberate.
