Cauchi was reported to have been medically managed off his medication by his GP and a non-government Psychiatrist, approx. 5 years ago (in 2019).

That's way too long a time ago to claim that this triggered him relapsing into a psychosis in April 2024.

We'll have to wait for the results of the toxicology report that should be revealed (at the latest) at his Coronial Inquest, for confirmation of what drugs were in his system at the time of death, for a clearer indication of what might have triggered a psychosis in the immediate lead up to the Bondi Junction tragedy.

How many psychosis relapses (if any), and the severity and length of them, he might have had since 2019 before the April 2024 psychosis, is unclear.

For all we know, if he did have any, they might have been been very minor ones, and have been "mutism" ones, where he was possibly of no-harm to anyone, but possible himself having an accident.


We don't even know if pre-2019, how many (if any) psychosis he had relapsed into since his claimed aged 17 ones, decades ago, and what the likely causes of this might have been (including things like sleep deprivation or drug usage).

Wouldn't be surprised if there are still many unanswered questions after the Coronial Inquest, as is often the case where mental health is a factor, and there is at least a 23 year period of mental health illness, treatment (or lack of possibly to some degree), incorrect treatment or ineffective medications, medication side effects (possibly long term), or other undiagnosed illness.

Bondi Junction Inquest

  • Directions hearing: Tuesday 12 November 2024 at 9.30AM
  • Hearing: Dates to be confirmed
  • Location: Court 1, Level 2, NSW State Coroners Court, 1A Main Avenue, Lidcombe NSW 2141.
The Bondi Junction Inquest will primarily be held in open court, meaning members of the public may attend the proceedings. Only select portions of the hearing will be live streamed, including Counsel Assisting's opening address, and the State Coroner's findings.

The hearing will generally run from 10am to 4pm daily.

About the inquest

The Bondi Junction Inquest will relate to the deaths of seven people during the incident that occurred at Westfield Shopping Centre, Bondi Junction on 13 April 2024.

The NSW State Coroner, Magistrate Teresa O’Sullivan, will be assisted by a team of solicitors from the NSW Crown Solicitor’s Office and barristers known as Counsel Assisting (collectively, the team assisting).

The State Coroner’s role is to establish in relation to each person:

  • Who died;
  • When and where they died;
  • The cause of death; and
  • The manner of death (which means the circumstances surrounding the death).
If the State Coroner considers it necessary or desirable to do so, her Honour may also make recommendations in relation to any matter connected with the death. For example, recommendations may be made in relation to public health and safety.
