Exactly - what if the ball moves the length of the ground in that time? What if the offending team kicks a goal before the penalty is applied? And finally, why isn't the penalty just that the 76th man can't be replaced?

I think the answer to all these questions is "Steve Hocking came up with the rule" but I'll wait for the official explanation.

Problem with all those compared to the current system is that it that the decision needs to be made immediately, which I'd argue increases the possibility of an error.
 
I thought the rule allowed for injury replacements after the 75th interchange had been made, with the stipulation being the injured player coming off cannot come back on. In this case, Shiels came off with a cramp which should qualify as an injury replacement. Watch the AFL scramble to cover their arses.

True of all umpiring decisions, except this one is monitored by someone whose job it is to watch (and count) the interchange.
