When will the media be actually held to account over reported stories that are factually inaccurate?

The media should have to provide public incident reports for all instances of reporting fabricated stories without fact checking. Journalists who haven't done due diligence should face a review and potential sacking.
Why?

Their job isn't to report facts. Their job is to produce content and generate clicks and views.
 
A major issue is the first 'journalist' to report a breaking story is often rewarded where if they wait and report it along with everyone else their clickbait headline may get lost amongst the others. So they quickly release a misleading clickbait headline for views and then when called out get indignant and blame their source saying they were misinformed.

No repercussions and no accountability. Essy street being a sports journo.
 
Where is the accountability? Just cos they're in the media doesn't mean that they should be allowed to make assumptions and then just say whatever they want without checking facts. I genuinely don't get how there's never any accountability for inaccurate stories.
When everyone stops engaging with them. No clicks, no retweets and stop feeding them.
