The prison system in the USA is seriously broken

I'd generally just stop at the USA is broken, let alone their penal systems.

Dentists are part of the NHS (when they offer it), but they haven't been completely free to all for decades now. They are free for children, pensioners and those on welfare benefits, and charges are capped for those in employment, who must pay. For example, a check up and clean every six months which requires no treatment has a minimal charge of around 30 AUSD. Standard treatment of a filling or extraction is heavily subsidised, and advanced work such as surgery is likewise greatly discounted. With an NHS dentist, no matter the extent of work required (crowns etc) the maximum fee for those in work is around 600 AUSD. Not perfect but not too bad. I would prefer dentistry to be fully free to all at the point of delivery also, but the system isn't egregious.

How does the nhs handle the grey area between need and want? Breasts enhancement for "mental heath", lap band surgery because you can't be bothered exercising, veneers on your teeth so you have a nice smile, .......

Cosmetic surgery can be available on the NHS in extreme cases, when physical traits cause extreme psychological distress e.g. children with prominent ears or extreme obesity in adults. Such matters are at the discretion of the G.P. who will recommend a surgical intervention. However, facelifts and the like are not eligible.
 
The scary part of the US is that they export their model for economics and laws. The worst part of the US justice system is the plea bargaining. You can get falsely accused, told they are looking at giving you the death penalty, offer a sentence and you take the best worst option and cut off your chance of ever getting out by admitting false guilt. The side is it makes it quicker for prosecutors to get people into prisons that are private enterprises who then have the prisoners work for 50 cents a day and use them like slave labour.

Not hard to come to that conclusion when you discover that politicians are wearing bullet proof vests because the laws allow protestors to carry automatic assualt rifles into the parliament.

Although one wonders if the response would have been the same if the protestors were young Afro Americans or Muslims.

If you are accused of something serious, you can lose your life savings in lawyer fees, or you can agree to plea guilty and get a lesser charge.

Slavery as penal servitude is still legal in USA and 4 states offer no remuneration, see 13th amendment to constitution.
