It does seem like a rule that needs adjusting - given the umpire ALWAYS goes tv review for a run out, why does it matter whether there's an appeal?

Because the umpires always refer to TV umpire on run outs unless its ridiculously obvious like both players down one end of the ground. Logically, the rule should be changed to just review anything close, like how they check every ball for a NB
