We definitely have one Doctor. It's named in the role list. Others will be Random Town Protective or Random Town.

Pirate and Survivor would mean no Executioner, Jester or Amnesiac.

As per the OP there is definitely at least one doc . Maybe they assumed there was more than one so looked elsewhere . They can only self protect once in this one so I hope they didn’t waste the self protect on a night with obvious other targets
