The review system needs to have some external trigger as well.
Somehow the goal umpire missed it hitting the post but if all the other umpires aren't in the right spot to see the small deflection, then a review won't be called for.
Needs to be somone looking at a replay that can quickly relay to the umpires that there needs to be a review
