They “self report “to the club doctor
Who is required to lodge with the AFL a player has self reported, when they ask who the player is , standard response “sorry due to patient/doctor confidentiality I can’t disclose the identity “

The AFL does it’s “due diligence “ by asking said player gets the support they need… rinse and repeat
